Wisdom Specialist Centre is committed to providing quality services to you, and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.
We have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act). The APPs govern the way in which we collect, use, disclose, store, secure, and dispose of your Personal Information. 1. Personal Information Collection
Personal Information is information or an opinion that identifies an individual. Examples include name, address, date of birth, email address, phone number, Medicare details, and health fund details.
2. Sensitive Information
Sensitive Information includes health and medical history, racial or ethnic origin, religious beliefs, criminal history, and sexual orientation.
We only collect Sensitive Information for the primary purpose of providing healthcare services, for directly related secondary purposes, with your consent, or when required or authorised by law.
4. Collection of Information
We collect Personal Information through patient registration forms, consultations, telephone calls, emails, website forms, referrals from healthcare providers, and Medicare or health insurance claims.
Where reasonable and practicable, we collect information directly from you.
5. Purpose of Collection
We collect your information to provide healthcare services, diagnose and treat medical conditions, communicate with healthcare professionals, arrange referrals, process Medicare and health insurance claims, manage business operations, and comply with legal obligations.
6. Disclosure of Personal Information
We may disclose your Personal Information to treating doctors, specialists, hospitals, allied health professionals, Medicare, health insurers, and where required or authorised by law.
We will not use your Personal Information for direct marketing without your express consent.
7. Release of Medical Records
Medical records will only be released when:
• Written consent is received from the requesting third party; AND
• Verbal consent is obtained directly from the patient; AND
• The consent is less than twelve (12) months old.
If consent is older than 12 months, new consent will be required. Records may be released without consent where required by subpoena, court order, or law.
8. Storage and Security
We take reasonable steps to protect your Personal Information from misuse, loss, unauthorised access, modification, or disclosure.
Security measures include secure electronic medical record systems, password protection, restricted staff access, and secure storage of physical records.
9. Data Breach Notification
If a data breach is likely to result in serious harm, we will assess the breach promptly and notify affected individuals and the OAIC in accordance with the Notifiable Data Breaches scheme.
10. Retention of Records
Under NSW law, adult medical records are retained for at least 7 years from the last date of service. Records of minors are retained until the patient turns 25 years old.
When records are no longer required, they will be securely destroyed or permanently de-identified.
12. Access and Correction
You may request access to or correction of your Personal Information by submitting a written request. Proof of identity may be required before information is released.
13. Website Privacy
Our website is for informational and educational purposes only. No doctor-patient relationship is established through its use, and no medical, dental, or surgical advice is provided. The information should be used in consultation with a qualified healthcare professional. We are not responsible for the content of any external websites linked to ours, nor does hyperlinking imply endorsement.

15. Policy Review
This Privacy Policy is reviewed regularly to ensure compliance with legal and operational changes. The latest version will be made available by Wisdom Specialist Centre.
