This Personal Information is obtained in many ways, including consultations, patient information forms completed by you, correspondence, by telephone and facsimile, by email, and via our website www.wisdomcentre.com.au, and from third parties. 1.2 How do we collect your Personal Information?

We collect your Personal Information for the primary purpose of providing our services to you, providing information to your doctors/health care professionals/hospitals, and referring you to other doctors/health care professionals/hospitals. We may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. When we collect Personal Information, we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

2. Sensitive Information

Sensitive information is defined in the Privacy Act to include information or an opinion about such information as an individual’s racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record, or health information.

Sensitive information will be used by us only:

  • For the primary purpose for which it was obtained
  • For a secondary purpose that is directly related to the primary purpose
  • With your consent or where required or authorized by law.

3. Third Parties

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances, we may be provided with information by third parties. In such a case, we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Your Personal Information may be disclosed in a number of circumstances, including the following:

5. Security of Personal Information

In order to protect your Personal Information, we may require identification from you before releasing the requested information.

We take complaints and concerns regarding privacy seriously. If you have any privacy concerns, please express them in writing. We will then attempt to resolve your issue(s) in accordance with our resolution procedure. If you have lodged a complaint with us and have not received a response within 14 business days or if you are dissatisfied with our response, you may escalate your complaint to the Office of the Australian Information Commissioner (OAIC). However, please note that the OAIC may decide not to investigate your complaint if you have not raised it with our practice first. 9. Direct Marketing

Our practice will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent to receive direct marketing communications, you may opt-out at any time by notifying our practice in writing.
